I'd say death in bleach is only meaningful when the spiritual body (the soul itself) dies. Similar to how demi-hollows are chained to places of past regret (places they depend on) we are all chained to our bodies. Perhaps the chain isn't even necessary and is simply a product of our natural reliance on our bodies to move around. Dying (as a human in the world of the living) in bleach is simply having your soul detached from it's physical body. It's kind of odd since there's no change that even normal souls go through upon death. One could argue that he IS dead and his body might as well be an advanced gigai made of living cells. Ichigo might be the only one who has done it without any inventions. Rukia's was just specially made to keep her powers from returning. And because they already have Shinigami powers, they can go in and out whenever they want. His special Gigais probably act as if they were the Shinigami's actual living human body.

This might be a similar case to how Kisuke's special Gigai can turn Shinigami into full Humans(like with Rukia and Isshin) or make Shinigami have Human-like bodies in order to interact with the Human World without restricting their powers(like Kisuke himself and the Vizards). This might allow him to go back to his Human body, as if he was never 'separated'. I think Ichigo might be a special case, since he already had spiritual powers before having his chain cut. And he needs his Human side in order for him to have a balanced soul. If he truly 'died', then he shouldn't be Human anymore.
